    Got tired of looking for this form after searching the interwebz, chatting with GM customer service, and finally visiting a dealership, I got it. Now it's time to make it easier for everyone else

    Fill out the form, include the requested documentation, and get a refund for your LCA kit!

    Only good for out of pocket expense...if it was covered by warranty, it doesn't count.
